possession of said premises, to declare said lease or rental agreement forfeited
and to recover rent and punitive damages as allowed by law.
YOU ARE FURTHER NOTIFIED that by this notice the landlord/agent elects to and
does declare a forfeiture of said lease or rental agreement if said breach is
not remedied within the three (3) day period. The premises herein referred to
